Welcome to Achieving Health. I'm Chad Mulvaney. And I'm Sean Stack. Thank you for joining us. Let's get started with today's Washington Watch. These updates reflect information as of June 19th, 2026, and are subject to change. As far as our agenda is concerned, HRSA is advancing the 340B Rebate Model through a series of regulatory steps, including a request for information, a pre-rule submission to OMB,

and a new information collection request aimed at addressing legal deficiencies that invalidated the original rebate model. Based on an information collection request HRSA recently released, the revised model is expected to broaden the scope to include drugs under Medicare price negotiation in both 2026 and 27, increasing exposure for covered entities and introducing significant operational compliance and data management challenges.

Also, from HFMA, the One Big Beautiful Bill Act is expected to raise the risk of credit rating downgrades for not-for-profit hospitals by reducing Medicaid coverage and ACA marketplace enrollment, potentially lowering revenue over time. While larger systems may adapt, many providers must proactively improve efficiency, adjust services, and implement credible mitigation strategies to withstand prolonged financial pressure.

In the last few weeks, healthcare price transparency has been brought back into the spotlight, with the administration, HHS, and CMS turning up the heat, warning that hospitals not meeting their expectations around compliance and engagement, will face escalating penalties and heightened public scrutiny. And in just a moment, I'll be joined by our colleague Victoria Duffel to walk through this and other price transparency topics that are top of mind for healthcare leaders as we head into the summer and fall of 2026.

And then finally, a quick note, on June 11th, the HHS Office of Inspector General released a report finding that Medicare Advantage plans overturned nearly all appealed prior authorization denials for skilled nursing facility admissions, raising serious concerns about the appropriateness of initial denials and how many unappealed cases may have resulted in delayed or denied care. I'll be diving into that report at the bottom of the podcast today.

And then the last thing Sean and I wanted to flag is that the deadline to apply or the application for HRSA funding for certain qualifying low-wage hospitals has opened. And this is funding that's a result of the Consolidated Appropriations Act of 2026, targeted to...
