Already the world's biggest selling drug thanks to its use in type 2 diabetes as Mounjaro and obesity as Zetbound, and with a sleep apnea approval added in 2024, Lily has now achieved success in two Phase 3 trials, together PSA and together PSO.

These saw the GLP-1 slash GIP receptor agonist help overweight and obese patients with psoriatic arthritis or psoriasis respectively when taken alongside its anti-IL-17A therapy TOLTS. Full results from together PSA were presented on June 3 at the European Alliance of Associations for Rheumatology conference in London, and Lily is now preparing to talk to regulators about both trials.

Together, PSA met its primary endpoint of a 50% improvement in PSA activity based on ACR50, in addition to greater than or equal to 10% weight reduction with concomitant TOLTS and Zetbound therapy at 36 weeks compared with TOLTS monotherapy. If approved, this expansion into autoimmune diseases would help boost Lily's presence in immunology and give it an advantage over rival Novo Nordisk,

which has no similar studies underway for its GLP-1 blockbuster, Wegevy. This foray into autoimmune diseases has been led by Lily's immunology development leader, Mark Genovese. Speaking to Scrip at EULA, Genovese said Zetbound's crossover into immunology was a priority as soon as he joined the company. The decision was based on substantial evidence for Zetbound's benefits beyond weight loss and type 2 diabetes and into cardiovascular outcomes,

and the fact that nearly two-thirds of PSA patients in the US are either overweight or obese. The full data presentation at EULA showed a host of encouraging clinical biomarkers with the combination, including significant improvements in the ACR50 and the ACR20, standard measures of 50% or 20% improvement in tender or swollen joints as early as week 4, even before patients saw meaningful weight loss.

Genovese said, Together, PSO psoriasis study also hit its primary endpoint of 10% weight loss and skin clearance measure PASI-100, and discussions with the US FDA are likely to take place in the second or third quarter.
