Oncology Brothers: Practice-Changing Cancer Discussions

Hello, and thank you for joining us. I'm Raval Ghossain, here with my brother and your co-host, Raval Ghossain. We're two community medical oncologists, and we are the Oncology Brothers. In our ongoing series of conference highlights, today we're touching on a few key studies in breast cancer from ASCO 2026. And for this, we're excited to welcome back Dr. Hope Rugo. Hope, thank you so much for joining us. Hope Rugo Thanks for having me.

Dr. Hope Rugo Welcome, Hope. We saw exciting stuff going into ASCO in breast cancer space that was with the approvals of T-DXd in neoadjuvant and adjuvant settings. And at ASCO, we saw some more updates. So let's dive right in. For our discussion today, we will cover two studies in localized hormone receptor positive space and then one in metastatic space. Following that, we'll shift gears into the triple negative breast cancer space.

Dr. Hope Rugo Starting out with our first study, that is the OPTIMA trial. To set the stage in our clinical practice for that early hormone receptor positive disease, we rely on tumor size and biology where we use recurrent score to decide on that adjuvant treatment option from chemotherapy standpoint, whether that's for premenopausal or postmenopausal. And we often worry about, are we over-treating some of our patients, especially when we know that in premenopausal, OFS could be good enough.

Dr. Hope Rugo In this OPTIMA study, we have a new test and a scoring system. Hope, can you please walk us through the study design and its findings? Dr. Hope Rugo Absolutely. I think arguably, this was the most practice-changing study that was presented in breast cancer at ASCO this year.
